Do I need Dataverse if I only use Business Central?

No. Business Central has its own separate database and application core — it does not run on Dataverse. You only need Dataverse if you also adopt a CRM-side Dynamics 365 app (Sales, Customer Service, Field Service) or build a custom Power App, and want that data connected to Business Central via an integration.
This is one of the most common architecture misunderstandings for people coming from the CRM side of Dynamics 365: Business Central, Finance and Operations, and the Dataverse-based CRM apps are three separate data platforms under one marketing umbrella, not one shared database.
If a Business Central-only company later adds Dynamics 365 Sales for a sales team, Dataverse arrives with that app, and the two systems then need an explicit integration (a first-party connector, Power Automate, or a custom build) to share customer and item data — it doesn't happen automatically just because both are 'Dynamics 365.'
